Geriatric Care - Geriatrics Primary Care Clinic
If a patient or family member would like to be seen by a geriatrics physician or nurse practitioner in one of the GRECC clinics, they need to be referred by someone within the VA at the time of entering the VA or by their regular VA provider. Typically patients are 65 years or older.
The Geriatrics Primary Care Clinic, also called the Geriatric, Evaluation and Management (GEM) Clinic, has over 35 years of experience in caring for older Veterans.
Involves non-institutional long-term support and our programs provide a continuum between inpatient and outpatient care. Geriatric care occurs through the GEM, Older Women's Health, and Community-Based nursing home outreach program.
Consultative care includes Osteoporosis, Gero-psychiatry, and Memory Assessment/Dementia Clinics.
Hospitalized older Veterans receive care through the hospital GEM and inpatient consultative services. Families are supported through our Caregiver Support Group and inpatient respite program.
Clinic care focuses on getting to know the “whole person” including their goals and preferences, their family and social circumstances and making decisions in partnership with the veteran and their caregiver (if one exists). GRECC professionals work to help patients maintain the highest level of function and best quality of life possible with the least degree of complications or disability from medical and psychological conditions.
The Geriatrics Research Education and Clinical Center (GRECC) at the William S. Middleton Memorial Veterans Hospital also manages a number of specialty clinics:
Geriatrics Primary Care - four half days a week
Geriatric Comprehensive Assessment Clinic - one half day a week
Osteoporosis Clinic - one half day a week
Geriatric Psychiatry Clinic - two half days a week
Memory Assessment / Dementia Clinic - three half days a week

Inpatient Geriatrics Consultation
The GRECC also has physicians who evaluate hospitalized older veterans and may sometimes assist with their care when they move to area nursing homes and rehabilitation centers.
